Cardiology Case Report: Dyspnea on Exertion Playing Golf

Our latest cardiology case report from Dr. Pregerson features a man in his mid-40s presenting to the hospital with dyspnea on exertion during golf the day before. Check out the ECG and determine the correct diagnosis!

History:

A patient in his early-40’s presents to the hospital with DOE while playing golf. Normally he walks 18 holes w/o difficulty but yesterday he struggled the entire time. He denies syncope, palpitations, chest pain, fever, cough, leg swelling or other complaints.

Exam:

Vital signs are normal. The physical exam is also normal.

An ECG is done:

EKG readout

Computer read: NSR at 70, ST & marked T wave abnormality, consider anterior ischemia

Other info: CXR, troponin, CBC, BMP all normal

What is the most likely cause of the ECG findings in this patient?
